You mention phone #'s, is it your telephone # and/or the number where you'll be staying?
Does anyone have a copy of questions asked so one can familiarize with this form? Where online? I have a technology deficit brain, and an A personality where as I like to prepare ahead.
Just for kicks, I decided to fill mine out today, even though we're not arriving until November 21. The city of my residence isn't on the drop-down list, and there's no way to type it in.
Carol, not that I saw. I did find our county on the list, so I just used that. When I finished, I got an error message that my passport needed to have at least three months left; it doesn't expire until 2026! I didn't see a way to edit that, so I went back and started all over, thinking I may have made an error entering the expiration date. When I finished, I got an error message that an entry already existed for my travel dates. I'm just going to wait a few days and try again. It's a very cumbersome form; I definitely don't suggest trying to complete it on a phone!
Edited to add that I received an email with an application number to retrieve my application. I did that, and there were no errors this time. I guess that means I'm good to go? I did get an email thanking me for completing the form.

OK, sounds like this thing has a lot of bugs left. I can't imagine how someone who is not very computer literate would be able to do it. I guess the answer is, they won't be able to. I haven't really been following this thing that much. Do you have to fill out this thing in order to check in?
I don't know the answer to that, but once you figure it out, it's not really that difficult. I don't think I actually got an error the first time, because when I filled out David's, after I submitted it, there was a section with requirements that you should have when you enter SXM. The 3-month passport requirement is there, and it's in red print, which led me to believe it was an error message. Interesting that they're saying that, as we all know that your passport only needs to be valid though your return date. Whatever, it's done now. And we don't travel for another month, but I was able to complete the forms today.
I filled out the form for my trip Nov. 2, got a Do not reply that they got it, but the doc number is so, so long. Am I suppose to get am QR code or something back from them. It keeps saying fil out everything with the red asterisk but I did all that and checked it a bunch of times. Does any one know?
You will get an email back that says "Sint Maarten Entry Form Registration For Your Upcoming Trip" and towards the bottom left it will show "Passenger Name" and the name of the person the application was for. That is all you receive in return.

That is very childish and mean spirited and would just add to any problems that other users would have trying to get registered and confirmed. How about trying to be constructive and affirming, it would go a long way.
How is the emergency phone # supposed to be formatted? I tried with a 1+the area code and phone number both with and without dashes and it wouldn't take.
The confirmation will come as an email that will look similar to the image below. Your email address in the top left where I blacked out and your name in the lower right where I blocked out under passenger.

Well, I actually was able to complete my registration form today, as well as that for my husband, so we're ready to fly. I'll try to ignore your nastiness.

Not for nothin', but is there some logic to the + in front of the phone number? Is that some techie coding deal? As it seems like an unnecessary complication of the form.
The + is a convention for international phone numbers. It precedes the Country Code. Alternatively, 011 can sometimes be used in place of the +, but the + is much more common.
