Is there anything required to travel back to the US other than negative Covid test? Someone said there are CDC forms that need to be filled out. If so, what are they, where are they and where and when do we get them and fill them out?

There is an attestation form to be filled out, if your airline has an app for it, American has Verifly and I believe Delta also has an app, then it is filled out in the app. We flew American in October and it was a very easy proccess, we flew Jet Blue this week and they don't have an app for it, they were handing them out in the line for check in and telling you where to check and sign

If you don't already have it, the airline will give it to you when you check in, to fill out. No big deal.

Delta sends you an email and asks you to fill out electronically . I had done that 2 days prior to returning to US. When we arrived at the airport a Delta rep announced anyone who hadn’t filled it electronically could scan the big QR coded paper to fill out otherwise they wouldn’t be able to check in at the counter. He repeated this throughout the 45 minutes we were on line. I have no idea what people did who didn’t have a phone? There was no paper to fill out as we had in April.

The new fillable form on the CDC website is now 5 pages since the new requirements for non-citizens went into affect earlier this month.

Returning from Aruba this Saturday, as Carol mentioned, it was a link in the "Ready to check in" email from JetBlue. Easy to fill out but not mentioned at all at the airport...even with US Customs and Immigration in Aruba (we were the only ones using Global Entry and avoided a decent line)

We just flew back on Tuesday. A JetBlue rep had the forms and was getting you to sign them as you stood in line to check in. My JetBlue email didn't have anything electronic to do.

Also, did the negative COVID test and no one even asked for it ANYWHERE! Weird.

When I flew back 11/4 JetBlue sent an email with the link to fill out the form. We went through security (no bags) we had to check in at the gate and they looked at my Covid test and stamped my tickets as “docs ok” and that was it. They had the attestation info already on my ticket in their computer.

We flew home yesterday on JetBlue. They sent us a online form by email but if not you can scan barcode while online at the airport and fill it out electronically. Btw - we arrived at airport 3 hrs prior and we needed that much time. It was busy on a Saturday afternoon.

We flew home yesterday (Saturday) and Delta sent us the information 4-5 days before the flight to upload everything to their 'FlyReady' site. Easily done. If more folks would do it, the airport experience would be MUCH faster. So many folks didn't bother and were filling it out at the airport - really slowed things down. When we got the counter, the agent thanked us several times for having uploaded the info the day before. We were curb to gate in about 25 minutes.

Amen with JB. 1:33 flt 1788 SXM to Boston direct. Arrived 3 hrs 15 min early counters not open. Acceptedf flights for Fl and Bos only. Got email from JB while in line for both of us. 9 times filled out and submitted @ airport on iPhone. Rejected 9times. Found a floor agent who gave us simple paperattestation Covid form. 3 ck marks and sign. All Done. Get to counter Surly agent demands iPhone attestation confirmation only. Told her of problem. Same demand. Lost is. Showed her simple paper form re advised and showed 9 x email fail. And ordered her to accept paper forms provide byawesome floor JB reps. Voila. Done. Down to gate 10 and the airport angels led me directly to Bar @ gate 9/10 . Arrived in Boston on time with 15 min late departure. Our gate occupied by lost flight. 15 min late to gate. Into global ck out and home before heavy rain and wind . Yes a lot of hoops both ways but the week was worth it imho 👏👏👍❤️
