It looks like you have to pay maybe $35.00 for mandatory health insurance? Is this done when you fill out the form for approval? Also, do you send in your negative Covid test with the form? How do you get them the test results? Do I just forward them the email when I get the results? Thanks

As you fill out the form, when you get to page two, you need to upload a document (not a picture) of your test result. Then, when you get to page 3, you pay the $30 for the insurance. Hopefully they will email you a pdf of the test result that you can upload to their site.

This might be a bit confusing but here goes....our results were in the testing portal and said all the good stuff BUT it was not in pdf form....soooo....I converted to a jpg via "snip" in Windows and then converted to a attachable pdf....sounds complicated but it worked.
Hopefully your results will be in pdf form and eliminate the geek stuff needed
Edit: I should add that uploading the pdf is self explanatory in the EHAS,,,,

Or, if they email you an attachment, it is probably a pdf (Adobe Acrobat). Save that document somewhere and when you do the application, browse your computer, phone etc. to find it and then click on it and it is uploaded.

Your test results might be emailed to you as a PDF attachment. If this is the case, save this document to your computer, and attach it when you're completing the EHAS website.
If you get your results online (MyChart), you can probably create a PDF (electronic) document. If you're using Windows, you can try this right here on this website. Move your cursor to a blank spot on the left, and right click. Select PRINT. Then select the DESTINATION as "Save to PDF." Save it to your computer, and attach to EHAS website. So when you're looking at your results in MyChart, select the PRINTER icon. Right-click on the document, and follow the instructions above.
If you're using a MAC or other Apple product, you need to ask somebody else.... I have no idea.
