Hi everyone! we will be trying to return to SXM later this month. Is there a downloadable form that I can get? Want to see what all the questions are so we can be prepared to answer. The form at the website must be filled out to progress to next page and be less than 14 days before departure. Thanks

There's no form or document to download. It's all online at the EHAS website. You'll use the website to upload your r-PCR test results.
So there's nothing to do until you get your test results (testing less than 72 hours before flight to SXM takes off).

You can complete page 1 of the application using a fictitious date and that will at least let you see page 2.

Does anyone know how the EHAS department is staffed on weekends? We'll be luck if we can upload our forms before Saturday morning for a Sunday flight and I'm wondering if we have issues will anyone answer that phone number on Saturday.

Yes, they have staff seven days a week; most of them are working from home.
