I just received a quote for my car rental and there is a surcharge fee of $48 in the quote. This is on top of a location fee and taxes. I don’t know if I’ve ever seen this before and wondered what it is. The total quote for nine days in January is $318 US without liability insurance. Does this seem reasonable?

Could be some airport surcharge fee. Who knows? Doesn't matter. That's why we always get a quote that includes everything, all taxes and fees and then compare prices. You can drive yourself crazy with these things. Best suggestion is to just get a couple of quotes and go with whichever one you feel is best.

Same as others have said. Ignore the line items and pay attention to the total quote. I am paying $144/week (three-week rental) for a compact for my upcoming trip. This price reflects "loyalty discounts."

Most of us have used the same rental car company for a number of years. Usually, that means we trust the people that run the company and have had good customer service when we needed it.
The variables that effect the quoted price are: Time of year, High Season and Low Season Size of car, sub compact, compact etc. Number of drivers Taxes Misc. fees and surcharges like the one your mentioned Insurance, Yes you will want to confirm that your credit card covers collision (damage to rental car), and liability insurance that can range from $5/day to $11/day Loyalty discounts
Some people have regretted not getting TOTAL quotes or checking with TTOL sponsors and getting unpleasantly surprised when they turned in the car before going home.

Yes. That second driver is an important feature in the rental. Another reason we stick with Hertz as the spouse is included as a fully covered driver in the rental quote.
