Some of these issues are already covered by policy. United downgraded me from EWR-LHR last year. They gave me an empty 3 seat row, refunded my miles, and gave me a $500 voucher. Pilots were deadheading to London and the union for Continental states that they go business when commuting overseas.

Regarding #12, how is it even legal for them to collect tax and not pay it to the government on cancelled tickets. Some loophole or something but yes, please fix it.
